Forms that match what you offer
The guest chooses what they plan to do. HALIBLU shows the questions, documents and equipment sizes configured for that activity.
A snorkeller sees no questions about dive experience. For certified divers, certification, last dive, insurance and equipment sizes can be required.

You decide what HALIBLU asks
Name, contact details, certification, dive experience, last dive, insurance and emergency contact can be requested for the relevant form variant. Document uploads and equipment sizes can also be shown or hidden per variant.
First name, last name and email always remain required, so every submission can be matched reliably.
Documents come straight from the phone
Guests upload their ID, certification, doctor’s certificate and proof of insurance as a PDF or photo. The files do not disappear into an email inbox; they arrive with the check-in for review in HALIBLU.
Returning guests do not start from scratch
With a personal login, existing details are already filled in. The guest confirms what is still correct and changes only the certification, documents, sizes or contact details that have changed.
The RSTC medical is built in
The medical self-declaration follows the RSTC form. Guests can complete and sign it digitally in seven languages. The form itself is standardised and is not changed by the dive centre.

Reception sees what has arrived
New check-ins and changes wait for reception to review them. Staff see the form variant, stay dates, documents, medical, guest details and equipment sizes together.

Reception compares existing and new details. Staff can accept changes or reject the submission with a reason.
Only confirmed details move into the central guest profile and become available for preparing dive operations.
